To validate the quit card presented to you, enter the quit code in the box below, and your pharmacy name in the next box. Clicking 'Validate' will check that the quit card has not been previously used - if it has then you will see a message in red to this effect and you should refuse to accept the card.

If the card has not been used then you will see a message in green to say that you may accept the card, and the system will log the card as having been redeemed by your pharmacy with today's date.
